If you need post-hospital home health care, your nurse or JCMH social worker will give you information about Home Health Services.

HomeCare services offer a full range of skilled medical and social services to support the physicians plan of treatment for health problems requiring extra attention.

HomeCare services include: administering home IV medications, pain control medications, nutrition information, physical and occupational therapy and medical social services. Services can be provided 24 hours a day, seven days a week.
